Collaboration between leisure providers and healthcare services is central to the effective support of people living with long term conditions. Innovation in the delivery of healthcare can optimise the transition from NHS based rehabilitation into third sector services. Leisure providers are extending their remit to support people with complex changes in experiencing exercise outside clinical settings. Understanding the challenges and benefits associated with examples of real world service developments will enable you to think about your local services and identify opportunities for supporting patients in achieving their activity goals in non-clinical settings.
