Collaboration between leisure providers and healthcare services is central to the effective support of people living with long term conditions. Innovation in the delivery of healthcare can optimise the transition from NHS based rehabilitation into third sector services. Leisure providers are extending their remit to support people with complex changes in experiencing exercise outside clinical settings.
