The future of health care will see digital tools, data and technology empowering the nation to help manage their own health. In the coming years digital services will become more wide spread – moving the management of health out of clinical settings and into the hands of patients.

This session will share the role of digital health tools in supporting patient care, examining what tools are available and how do they support patients, how these tools can be effectively integrated into care – and the practicalities of introducing such tools. And will explore how digital technology can be used to offer precise, personalised and targeted health advice and support.
