Personalised care and social prescribing

Social prescribing is a key component of NHS England’s drive to champion ‘personalised care’ – and has seen great innovations in health and wellbeing approaches outside the standard medical model. Recognising that most health issues are not associated with access to healthcare, social prescribing helps people improve their health and wellbeing by connecting them to community services run by a range of stakeholders. Emerging evidence demonstrates a range of positive health and wellbeing outcomes and indicates potential reductions in the use of NHS services and/or more appropriate use. With physical activity, specifically reducing inactivity, continuing to play a vital part in social prescribing, this session demonstrates best practice and innovative approaches.
