Personalised care and social prescribing

Social prescribing is at the core of the NHS drive to champion ‘personalised care’ – and has seen great innovations in non clinical approaches. Social prescribing helps individuals to improve their health and wellbeing by connecting them to community services run by a range of stakeholders. Emerging evidence demonstrates a range of positive health and wellbeing outcomes and indicates potential reductions in the use of NHS services – with 59% of GPs thinking that social prescribing can reduce their workloads. With physical activity continuing to play a vital part in social prescribing this session demonstrates best practice and innovative approaches.
